1 New Patient Intake Option 1

The New Patient Intake Option 1 includes the initial intake appointment with two follow up nutrition consultation sessions. This is for self-paying patients. If you have health insurance, be sure to contact us first to confirm benefits.

To become a patient of Artemis in the City and Dr. Heard, you must complete a new patient intake assessment.

The New Patient Intake includes a thorough health history review, dietary recall, questionnaires, review of your health goals and what would like to achieve through working with a clinical nutritionist as well as review of a recent comprehensive bioscreen diagnostic lab test.

Comprehensive bioscreen diagnostic lab test results are required in order to complete the nutrition intake assessment. Lab tests can be ordered through your medical doctor or Artemis in the City and are not included in the listed cost.

Nutrition Consultation Session

A Nutrition Consultation is only available for current patients and is for follow up appointments for evaluation and monitoring, nutrition therapy and test reviews. This is for self-paying patients. If you have health insurance, be sure to contact us first to confirm benefits.

This is 1 one-on-one 1-hour (60 min) nutrition consultation session scheduled with Dr. Danielle Heard, DCN, MS, MS, CNS, LDN, CDN, CKNS, INHC.

A nutrition consultation session can include recommendations based on nutrition assessment and food diary findings, specialized diet recommendations, review of diagnostic testing results, recommended nutrition therapy where necessary, health and nutrition handouts and recipes. During nutrition consultations Dr. Heard works directly with you to help you make nutritional and lifestyle changes.

Program Includes:

One 1-hour (60 min) nutrition consultation session scheduled with Dr. Danielle Heard, DCN, MS, MS, CNS, LDN, CDN, CKNS, INHC.

The nutrition consultation session must be paid for in advance in order to reserve the session time in Dr. Heard's calendar. If for any reason you cancel without 48-Hour notification, the payment is non-refundable. If you have a personal or family emergency we will work with you to make up the session. Nutrition Program 3 Month

This 3 month nutrition coaching program includes 6 one-on-one 1-hour sessions within a 3 month period with Dr. Danielle Heard, DCN, MS, MS, CNS, LDN, CDN, CKNS, INHC. This program is only for patients who have completed a new patient intake. This is for self-paying patients. If you have health insurance, be sure to contact us first to confirm benefits.

This ongoing coaching program is designed to provide support to patients as they make positive nutrition and lifestyle changes to feel good, lose weight, heal from illness, make better food choices, create a balanced healthy life and reach desired goals.

The sessions are held every other week for 3 months to keep patients focused on meeting health goals, implementing recommendations and to provide ongoing health and nutrition education. New handout materials and recipes are provided with each session. This 3 month nutrition coaching program is offered to individuals only.

The 3 month nutrition coaching program must be paid for in advance in order to reserve the session time in Dr. Heard's calendar. If for any reason you cancel a session without 48-Hour notification, it is non-refundable. If you have a personal or family emergency we will work with you to make up the missed session.
